>Coordinate care with other healthcare providers, including physicians, therapists, and social workers.
>Communicate regularly with the patient’s physician to ensure continuity of care.
>Maintain accurate patient records and documentation.
>Provide emotional support to patients and their families.
Requirements:
>Ability to work independently and make sound clinical judgments.
>Flexibility and adaptability to changing patient needs and schedules.
>Basic computer skills for documentation and communication.
